I hereby state that I have given serious thought to owning a pet. I understand that owning a pet is a wonderful experience, but also a responsibility and a commitment.

I plan to provide this pet with all the care necessary to give it the best chance at a happy and healthy life. This includes annual exams, vaccinations, dental care, and further veterinary care in the event of an illness or injury.

If not already spayed or neutered, I, the adopter, agree to have the pet/s spayed/neutered at the age of five (5) months. This is mandatory. I, the adopter, also agree to keep the pet/s as indoor pet/s.

I have read the prescribed health care regimen and agree to abide by it.

I understand that if these agreements are not being met, Bethayres Adoption Program retains the right to reconsider this adoption and reclaim ownership of the pet/s.
